author(s): possin, katherine l; moskowitz, tacie; erlhoff, sabrina j; rogers, kirsten m; johnson, erica t; steele, natasha zr; higgins, joseph j; stiver, jordan; alioto, andrea g; farias, sarah t; miller, bruce l; rankin, katherine p | abstract: background/objectivesbrief cognitive screens lack the sensitivity to detect mild cognitive impairment (mci) or support differential diagnoses. the objective of this study was to validate the 10-minute, tablet-based university of california, san francisco (ucsf) brain health assessment (bha) to overcome these limitations.designcross-sectional.settingucsf memory and aging center.participantsolder adults (n = 347) (neurologically healthy controls (n = 185), and individuals diagnosed with mci (n = 99), dementia (n = 42), and as normal with concerns (n = 21)).measurementsthe bha includes subtests of memory, executive function and speed, visuospatial skills, and language and an optional informant survey. participants completed the montreal cognitive assessment (moca) and criterion-standard neuropsychological tests. standardized structural 3t brain magnetic resonance imaging was performed in 145 participants.resultsat a fixed 85% specificity rate, the bha had 100% sensitivity to dementia and 84% to mci; the moca had 75% sensitivity to dementia and 25% to mci. the bha had 83% sensitivity to mci likely due to ad and 88% to mci unlikely due to ad, and the moca had 58% sensitivity to mci likely ad and 24% to mci unlikely ad. the bha subtests demonstrated moderate to high correlations with the criterion-standard tests from their respective cognitive domains. memory test performance correlated with medial temporal lobe volumes; executive and speed with frontal, parietal, and basal ganglia volumes; and visuospatial with right parietal volumes.conclusionthe bha had excellent combined sensitivity and specificity to detect dementia and mci, including mci due to diverse etiologies. the subtests provide efficient, valid measures of neurocognition that are critical in making a differential diagnosis.

the aromha brain health test is a remote olfactory assessment to screen for cognitive impairment - scientific reports
cost-effective, noninvasive screening methods for preclinical alzheimer’s disease (ad) and other neurocognitive disorders remain an unmet need. the olfactory neural circuits develop ad pathological changes prior to symptom onset. to probe these vulnerable circuits, we developed the digital remote aromha brain health test (abht), an at-home odor identification, discrimination, memory, and intensity assessment. the abht was self-administered among cognitively normal (cn) english and spanish speakers (n = 127), participants with subjective cognitive complaints (scc; n = 34), and mild cognitive impairment (mci; n = 19). self-administered tests took place remotely at home under unobserved (among interested cn participants) and observed modalities (cn, scc, and mci), as well as in-person with a research assistant present (cn, scc, and mci). olfactory performance was similar across observed and unobserved remote self-administration and between english and spanish speakers. odor memory, identification, and discrimination scores decreased with age, and olfactory identification and discrimination were lower in the mci group compared to cn and scc groups, independent of age, sex, and education. the abht revealed age-related olfactory decline, and discriminated cn older adults from those with cognitive impairment. replication of our results in other populations would support the use of the abht to identify and monitor individuals at risk for developing dementia.
